How sperm freezing works

  • Create an account with Fertility Options. 
  • The required consent forms will be sent to you automatically to be completed with a fast and easy e-notary process. 
  • The payment information you provide will be used for the initial collection fee and ongoing storage fees. You will not be charged until your sperm is frozen. 
  • In the portal, you’ll have access to resources from medical experts on sperm freezing and fertility preservation like the American Society of Reproductive Medicine (ASRM) and the Society for Assisted Reproductive Technology (SART).

  • You will receive a link via email to schedule a collection appointment after you have completed enrollment. 
  • Confirm that you are near a collection location. All clients must be able to visit the collection location in person. 
  • Select a day and time that works for you from our calendar of available appointments. 

  • You will be asked to remain abstinent for a period of time prior to your appointment. This is important to ensure a healthy sample. 
  • Arrive for your appointment at the collection location. 
  • You will be guided to a private room where you will produce a semen sample. 
  • Once you collect your sample in the cup provided, our experienced laboratory staff will take the sample and perform the freezing (cryopreservation) process. 

  • Your collection report will be available within 3-5 business days through our Fertility Options portal.

  • The collection fee and ongoing storage fees will be automatically charged to the payment method you provide at enrollment. 
  • The storage fee is charged monthly.

  • If you decide to use your frozen sperm for assisted reproductive technologies (ART) like in vitro fertilization (IVF) or intrauterine insemination (IUI), we can assist with the process of moving your frozen material to your physician’s office. Transport fees may apply.

  • If you decide you no longer wish to store your frozen sperm, we can answer any questions you may have. The online portal will allow you to access and complete any necessary consent forms through an easy e-notary process.

Sperm banking, or sperm cryopreservation, involves collecting, freezing, and storing sperm for future use. This is often done to preserve fertility before medical treatments or for individuals that wish to delay parenthood.

A semen sample is collected at a collection location.

Infectious disease screening involves testing blood and urine samples for various infectious diseases. This screening typically includes tests for sexually transmitted diseases such as HIV, hepatitis B and C, chlamydia, and syphilis. It is usually conducted around the same time as sperm freezing.

Consult your physician about recommendations regarding infectious disease screening. Once the necessary tests are determined, your physician can order the appropriate screenings for you.  Fertility Options services do not include infectious disease screening. 

Before cancer treatment or other medical interventions. When it comes to your health, so much can feel uncertain. Freezing sperm is a powerful way to take control and protect your future options.

Cryopreserved sperm can be stored for many years and later used in reproductive treatments such as intracervical insemination (ICI), intrauterine insemination (IUI), or other in vitro fertilization (IVF) procedures.

If you decide you no longer wish to store your cryopreserved sperm, the Fertility Options online portal offers a simple and convenient e-notary process to complete a disposition directive. Once your disposition directive is finalized, storage billing will cease. 

Some insurance plans provide coverage for sperm freezing and storage based on medical necessity. You will need to work with your physician and insurance company to determine if you have coverage available. 

Fertility Options is not contracted with any insurance providers.  Fertility Options can provide an itemized receipt for you to self-file a claim with your insurance provider if your insurance provider allows self-filing. 

A urologist or reproductive endocrinologist can provide you with more information on sperm freezing and male fertility.
